  4. Documentation for the party whose name is to be changed, either in its original form or in a copy that has been authenticated by a third party and which satisfies all of the criteria outlined in the table below. Replicas of the relevant documents must be authenticated as an exact reproduction of the original document by a Justice of the Peace or a Solicitor.   The documents that are produced need to be up to date, with the exception of an old Australian passport that has not been cancelled and was valid within the most recent two years before it expired.

When you submit your documents for lodging, you will be required to pay a fee at the following locations:

Note:

  • It is mandatory to present a marriage certificate issued by the Registry of Births, Deaths, and Marriages. There is no room in this category for a ceremonial marriage certificate. supportive evidence
  • Text written in a language other than English must be accompanied by a translation into English on any official document. The translation requires a signature on it. each page must be interpreted by the translator in order to be considered a reliable representation of the document.

Identifier Declaration

In the event that categories 1 through 4 are unable to be satisfied, category 5(a) may be utilized in conjunction with an Identification Declaration.

The identity of the person being referenced can be verified with the help of an Identifier Declaration. Original or certified documents must be presented in order to verify the declarant's identity. copies of any and all supporting documentation that satisfies the criteria laid out in any of the categories numbered 1 through 4 in the aforementioned table

The form that the Identifier Declaration takes is that of a Statement under penalty of perjury containing the following information:

  • the name and address of the person who is declaring their identity; and
  • occupation of the person whose identity is being declared; and
  • date of birth of the person claiming their identity; and
  • the character of the relationship between the Identity Declarant and the Third Party the Person Who Is Currently Being Identified; and
  • that the person claiming their identity does not have a family relationship with the Person Being Identified; and
  • that the person who is declaring their identity is not a party to the The Person Being Responsible for the Conveyancing Transaction(s) Identified either already has or is in the process of entering; and
  • the amount of time that has passed since the identity declarant known the Individual Who Is Currently Being Identified; and
  • the Identity Declarant affirms, to the best of his or her knowledge, based on the information and the beliefs concerning the Person Who Is Being Identified is who they assert themselves to be

If the requirements of category 5(a) cannot be met, then category 5(b) might apply. be used It is required to make the Identifier Declaration. by a person who is qualified as a Legal Practitioner in Australia, a Community Leader, a Court Officer, a Manager of a Bank, etc. Licenced medical practitioner, officeholder in the Land Council Attorney, Local Government Officeholder, and Conveyancer Nursing Assistant, Public Servant, or Law Enforcement Officer
